- Successfully managed a caseload of 24-26 patients with a multitude of complex psychosocial, physical, and symptom management needs while also attending to often difficult family dynamics and family members in various stages of grieving.
- Acted as a liaison between the patient, family members, hospice staff, and physicians regarding treatment options and/or prognosis updates as well as a patient advocate during bi-weekly interdisciplinary meetings and as needed for any status changes.

- Conducted regular visits with assigned patients in their home setting or at long-term care facilities to assess health status changes and on-going needs.
- Assisted my team with coverage for the on-call position while our on-call nurse was on maternity leave.
- Developed effective communication strategies with families and provided emotional support using active listening throughout the course of illness.
- Responded promptly to emergent situations involving hospice patients and provided crisis intervention when needed.
- Proficient in Home Care Home Base documentation system.
- Supervised LPNs and hospice aides per Medicare and agency guidelines to ensure patient satisfaction and compliance with standards of care.
