I have been working in residential care for many years and I have found this to be very rewarding and a great way to assist in implementing my clinical education and experience. My professional desire is to assist individuals in residential facilities to have the best quality of life possible. I look forward to working with other professionals as a team approach model and accessing outside provider services as well.
Implement Sub part S Psychiatric Rehabilitation Services directed by Illinois Department of Public Health
Daily case management services for residents of varying ages and diagnoses with psychiatric and neurologocal impairments
Coordinate individual and group psychiatric services with other staff members on a daily basis
Supervise Psychiatric Rehabilitation Services Coordinator staff within my department
Access provider services necessary for testing, evaluation, and support
Review and assist in the admission process for new residents
Weekly coordination of care with the Psychiatrist
Act as an assistant to the supervisor for the Adapt of Illinois Program
Coordinate monthly psychiatric medication management services with the Psychiatric provider
Supervise Qualified Mental Health Professionals within the Adapt program
Monthly attendance at managerial meetings with other supervisors for Adapt programs located at other facilities
Daily submission of clinical billing services under Medicaid Rule 132
Daily clinical documentation of individual and group services
Provide management services in residential care
Daily direct involvement with residents, caring for their daily needs
Involvement in treatment and care planning with other staff, families, and legal representatives
Access state guardianship services and assign Power of Attorney for Healthcare
Work with all departments in the facility to coordinate residenital care daily
Assist in obtaining and managing facility donations
