• I am currently providing primary care services in a state inpatient psychiatric hospital. In this setting, due to the acute mental health issues the patients are experiencing, It is more challenging to meet the physical needs such as an examination, obtaining lab tests, communicating with the patient in regard to their health issues, and having them as a participant in their care.
• I am responsible of 1 Admission unit of 28 patients with assigned cross overage of another 28 patients which can be an admission unit or a long-term unit.
• we can average 2-4 admissions, discharges, transfers in a day.
• due to being a state hospital we have seen an increase in Forensic patients at about 85% of our population now.
• there is a lot of acute injuries related to the psychotic, aggressive nature including fall risks, head trauma, intracranial bleed, Hand fractures, cardiopulmonary issues, among others.
• I work as part of the psychiatric team in, a collaborative manner, to bridge the holistic health of the patient in mind and body.
• As part of the Primary care department and of the Hospital staff I participate on 2 committees for quarterly monitoring of quality improvement items. For me this includes Morbidity, Diabetes, Hypertension, Asthma, COPD measures.

I worked as a Locum at an Indian Health Service Clinic in Anadarko, OK on a Reservation with multiple tribes represented.

My schedule would typically be 20 to 25 patients a day.

on site we also had Commissioned Corp. of Public health Service who aided with social service needs of the Native Americans.

• I provided family medicine care in a small rural community as the main provider on site. Managing age group from infants to elderly. This included preventative and acute care issues from immunizations, chronic health, urgent issues. Urgent issues could range from respiratory distress, acute coronary syndrome, injuries, among others. I completed testing and received Certified Medical Examiner status for FMCSA 4/24/2014. But since My current position hasn't required this I will need to update this.

• I provided primary care in a Veterans Administration community Outpatient Clinic managing Veteran healthcare. This would include history and physicals, management of their service connected disabilities, consult management to specialty services at one of the VA hospitals, lab work interpretation and utilizing the information to monitor ongoing health and identifying abnormal changes needing addressed.
• I also work work with other departments within the VAMC for support services.
• I served as the Clinical Manager for the last 6 months of the Facility contract. Due to every 5 years the VA would rebid the contract clinic agency. This included on site facility management from monitoring the cleaning service, staff scheduling, preparing the monthly reports the went to STG and Martinsburg VAMC, reviewing patient complaints and concerns, and adjusting to unforeseen issues that affected care assess such as loss of provider staff and working to replace providers.

• I provided family medicine care at this rural clinic at a time of transition. I managed infants to geriatric patients, acute/urgent medical issues such as acute coronary syndrome, respiratory distress, simple suturing, foreign body removal, Incision and drainage, splinting, among the other varied presentations.
• I would provide some home visits to elderly patients with transportation issues.
• Administratively, I would provide the yearly training requirements such as bloodborne pathogen, HIPPA, etc.. I took lead on the OSHA requirements for the clinic working along with the Clinic administrator.
• I was the lead on the Health disparities collaborative in 2006. As part of this I worked with our EHR provider and the collaborative to interface the system for data collection.

This was a rural Health center in a small town initially designated as a Rural Health Clinic and during my time there became a FQHC designated clinic in cooperation with Tri county Health Center.

• I provided Family medicine care along with acute/urgent care needs. Due to the distance from a local hospital, and the time to have ambulance services, I would mange the patient along with the medical assistant, who was a paramedic, until the patient could be transported.
• I worked with the local timer industry for their employee health needs. Completed examinations for the railroad workers, and DOT exams for truck drivers and school bus drivers.
• Administratively, I would work with the office manager in completing UDS reporting, we conducted quarterly Life cycle audits for Quality improvement measures.
• I worked with the Medical Assistant in utilizing the Patient Assistant programs to obtain needed medications for patients.

• I was the first Physician Assistant to work at this small, Rural hospital outside of Knoxville.
• We would average about 80 patients a 12hr shift.
• Enhanced overall clinic workflow by proactively identifying opportunities for improvement and implementing necessary changes.

• I was a sole primary care provider at the small, rural clinic located an hour south of Huntington in the coal mining town.

care included WV well child checks, Family planning, Breast and cervical cancer screening, WV vaccine program, disability physical exams for Medicaid, along with the chronic health care, preventative health care, DOT physicals. I also worked with the Black Lung program and when we built the new clinic we were able to have x-ray on site as part of the Black lung screening.

• I participated in a early Telemedicine development program in conjunction with WVU.
• We also were in collaboration with Marshall University and WVU public health for a Diabetes study.

as part of community out reach I collaborated with local stakeholders for annual health fairs, sports physical clinics, I participated on the Crum community improvement board and lead a grant to install a walking track at the local high school to promote safe areas for exercise and part of improving health outcomes for chronic care issues such as Diabetes.

I participated in a Community Leadership development course provided by the Brushy Fork Institute at Berea college in Ky.As/li>

• as part of the Valley health providers I participated with the quarterly Life Cycle audits and monthly meetings
• I would cross cover at sister clinics when needed

• I was the first Physician Assistant to be a member of the WV Medicaid Rational Drug Therapy Board

• This was a correctional Medicine position in Pennsylvania at a super max state prison SCI-Greene in Waynesboro and some time at SCI-Pittsburgh.
• I was responsible for sick call issues of inmates along with chronic care management and urgent care needs.
• This was fast paced due to the number of inmates that would need evaluated.
• During my time there I working with my supervisor in developing the fledging Hepatitis C Clinic and procedures for evaluation and referral to Gastroenterology
• I also assisted with the HIV clinic
• We had a Physical therapy provider who would come on site and I would assist with the PT plans so the patients could meet treatment goals.
