• Provide daily living support and personal care to individuals with disabilities in a residential setting
• Assist with activities of daily living, such as dressing, bathing, toileting, and eating, while maintaining privacy and dignity
• Implement individualized care plans and behavior plans, and document progress and concerns for each client
• Collaborate with interdisciplinary team members, including nurses, therapists, and case managers, to ensure continuity of care and optimal outcomes for clients
• Facilitate recreational and social activities that promote community inclusion and engagement for clients
• Administer medication and monitor client health, reporting any changes or concerns to the appropriate healthcare professionals
