Educating patients, especially those newly starting dialysis about End-Stage Renal Disease (ESRD), treatment options, including home dialysis modalities like Peritoneal Dialysis(PD) and Home Hemodialysis (HHD). Conducting educational sessions such as 1:1 or group classes, while working with the home managers and teammates to broaden the home modality knowledge with the region. Facilitating smooth transitions in treatment, such as from in-center to home dialysis in coordination with the interdisciplinary teams. Supporting clinics through community outreach or Kidney Smart Classes and CKD awareness initiatives, while working closely with the different Physicians, hospitals, nurses, social workers, dietitians and administrative staff. Tracking and reporting key performance indicators like the number of patients being educated, patients who choose or transition to home modalities, clinic visits and education sessions.
Modality education both PD and HHD. Operate NxStage cycler (HHD) and
Home choice Claria machine (PD). Monitoring patients as they undergo
dialysis. Maintaining, assembling, and cleaning dialysis equipment
Reinforcing patient’s education in line with modality training. Drawing labs
for both PD and HHD patients Operating Centrifuge. Evaluating and reporting
findings Reinforcement of cannulating AVF, AVG and CVC care
Reinforcement of medication preparation and administration on the prescribed
route Checking exit site for PD patient, insuring site is clean and clear of
infection. Checking in on patients to make sure PD treatments are being done
correctly
