Assessment of patients' and families' strengths and limitations with regard to response to rehabilitation and discharge needs.

Worked at Seacrest Village July, 2016-October, 2016 covering a maternity leave. Was invited back in June, 2017 when that employee left the facility.

Duties included:

Complete Adult Medical Day Center social assessments; develop behavior plans for attendees.

Address resident and family concerns in a timely manner.

Lead all other appropriate and necessary social service interventions.

Assess patient/family psychosocial strengths in adjustment to illness-primarily dementia and cancer; preparation for death; provide counseling, support and education where indicated.

Provide holistic, family-centered care across treatment settings to improve quality of life.

Support the patient's and family's spiritual and cultural beliefs.

Assist patient/family in utilizing community resources.

Participate in interdisciplinary meetings to establish, review and review plan of care for each patient.
