Responsible for everchanging patient caseload of terminally ill patients in their homes and long-term facilities. Evaluate potential patients for hospice eligibility, complete assessment and admission process, designate frequency of skilled nurse/hospice aide visits. Work closely with certified nurse practitioner and hospice medical director, hospice protocols, to ensure appropriate comfort medications are prescribed and therapeutic.
