• Develops, implements, and evaluates policies and procedures.
• Provides for consultation and training in identification, documentation and resolution of job performance issues, including substance abuse and treatment alternatives.

• Provides for clinical services for intervention, counseling, referral, follow-up, and case management of identified troubled employees based on approved protocols. Clinical services also include, but are not limited to: appropriate collection of specimens, review of specimen results, and physical assessments.

• Coordinates the Employee Intervention program with the Employee Assistance Program, department directors and human resources in the management of troubled employees.
• Participate in the development of QI/QA standards, assessing regularly and planning activities to insure quality services.
• Case manage the on-the-job injury program providing appropriate assessment and treatment of job related injuries/illnesses/exposures while ordering diagnostic screenings, labs and x-rays as defined by departmental protocols. Makes referrals for additional treatment, tests and screenings under the Medical Director's guidance.
• Responsible for tracking and trending on the job injuries and illnesses.

• Navigate patients through the complexities of the discharge plan
• Educate patients and/or care givers in understanding the patient's discharge diagnosis, medications, and treatment plan
• Coordinate care of the discharged patient with other health care providers
• Proactively identify opportunities for services and support based on specific needs and connect patient with needed resources
• Advocate for the needs of the discharged patient from the hospital setting
• Remotely monitor patients in the home setting to help decrease hospital readmissions
• Communicate patient status changes to the Provider allowing for medical changes to be implemented early allowing patients to recover in the optimal setting of the home

• Maintain collaborative communication with all physicians regarding quality metrics, goals, and opportunities for improvement
• Ensure compliance with policies and procedures regarding accreditation guidelines including, but not limited to FPPE and OPPE

• Conduct clinical chart reviews for data collection for The Joint Commission requirements, special quality projects, and the Medical Staff Performance Improvement Committee
• Assist the Physician Committee Chair of the Medical Staff Performance Improvement Committee with the coordination and prioritization of peer review activities and assist the reviewers with variation analysis and summary reports

• Devise strategies to enlist medical staff input into the organization's quality initiatives

• Develop strategic plans for the chest pain certification program through guidelines set forth by the Society of Cardiovascular Patient Care
• Member of multiple committees, including buy not limited to, AMI/STEMI, Magnolia Management Model (M3/LEAN), Medication Management, Patient Experience, and Provision of Care
• Process Improvement utilizing the LEAN approach within all facets of the Emergency Department
• Responsible for planning and analysis of departmental resources including budget, staffing, equipment, and productivity

• Coordinate patient care between a variety of ancillary services and settings
• Working knowledge of Medicare parts A&B, Medicaid, other payor sources, and DRG assignments using InterQual Data
• Acts as a case coordinator consultant to all disciplines and coordination of discharge planning
