Registered Nurse with 27 years of experience with diverse experience including direct patient care in the settings of ICU, surgery, cardiology, and emergency care enhanced by leadership experience in the areas of emergency care, case management, quality, and employee assistance.
• Navigate patients through the complexities of the discharge plan
• Educate patients and/or care givers in understanding the patient's discharge diagnosis, medications, and treatment plan
• Coordinate care of the discharged patient with other health care providers
• Proactively identify opportunities for services and support based on specific needs and connect patient with needed resources
• Advocate for the needs of the discharged patient from the hospital setting
• Remotely monitor patients in the home setting to help decrease hospital readmissions
• Communicate patient status changes to the Provider allowing for medical changes to be implemented early allowing patients to recover in the optimal setting of the home
• Maintain collaborative communication with all physicians regarding quality metrics, goals, and opportunities for improvement
• Ensure compliance with policies and procedures regarding accreditation guidelines including, but not limited to FPPE and OPPE
• Conduct clinical chart reviews for data collection for The Joint Commission requirements, special quality projects, and the Medical Staff Performance Improvement Committee
• Assist the Physician Committee Chair of the Medical Staff Performance Improvement Committee with the coordination and prioritization of peer review activities and assist the reviewers with variation analysis and summary reports
• Devise strategies to enlist medical staff input into the organization's quality initiatives
• Maintain confidentiality of case files, review findings, and coordinate external case reviews per policy • Ongoing support for analytics database and assist users in optimization of database
• Assist the Medical Staff credentialing office with required initiatives for onboarding physicians and physician reappointment
