  • This process involves evaluating individuals receiving services (IRS) with DD, along with evaluating requested or provided health services records, and comparing them against the submitted Individual Habilitation Plan (IHP) or Individual Support Plans (ISP) in relation to the investigation report and supporting evidence/documentation.
  • I also will evaluate the DD IRS plans and the agency’s clinical or behavioral health service records to ensure that accurate investigations are conducted and that relevant health records are collected and attached to the report.
  • I also request additional information regarding IRS with DD service plans; this process enables us to oversee the implementation of the IRS' ISP/IHP by monitoring the requirements for the safe and effective care of the IRS by the provider within the Community.
