Evidence-Based Project: To reduce 30-day readmission rates among Acute Heart Failure Exacerbation patients we developed and implemented a CHF care plan including a Heart Failure patient education folder in 3 languages, Heart Failure supply bag including scale, pillbox, ect, and scheduling a follow-up appointment within 10 days of discharge with the patient's cardiologist or the heart failure clinic. This project was a multi-disciplinary effort including nurse educators. heart failure NP, IT staff, and EPIC informatics specialists.
