Nurture resident and family relationships and build excellent working relationships with an array of medicaid staff and physicians.

Accountable for 90% of admissions and the coordinations of patient discharge planning needs.

Experience in the use of Care tracker and Answers on Demand(AOD) programs for charting purposes.
