Perform acute hemodialysis treatments in ICU, telemetry, and med-surge unit

1:1 or 2:1 Hemodialysis treatment provided to AKI/ESRD/kidney transplant patient in acute hospital system

Documentation Pre-/Intra-/Post-assessment via EPIC. Collaboratively working with nephrologists, primary RN, and other healthcare providers. Educate patient and family members about hemodialysis treatment, renal diet, fluid restriction, other modalities on dialysis, access care. Provide education about transition of care when patient is about to get discharged from hospital.

• Continuous Kidney Replacement Therapy (CKRT) Assess, initiate, discontinue, and monitor CKRT treatment (Prismax) on critically ill patients. Assist ICU primary RN for CKRT troubleshooting Documentation via EPIC. Collaboratively working with nephrologists, primary RN, and other healthcare providers Provide education to patient and family member about CKRT treatment, access care, transition into hemodialysis treatment or peritoneal dialysis
• Peritoneal Dialysis (PD)
  Assess, connect, disconnect, and monitor patient on PD (Both CAPD & CCPD).
  Education primary RN for emergency disconnect and basic troubleshooting on PD machine
  Documentation via EPIC
  Collaboratively working with nephrologists, primary RN, and other healthcare providers Provide education to patient and family member about PD treatment, access care, renal diet, fluid restriction.

• Supervised nursing staff to ensure compliance with safety protocols, enhancing patient care quality and operational efficiency.
• Coordinated patient care plans with interdisciplinary teams, facilitating improved health outcomes and streamlined communication.
