• Collaborated with physicians, care management as well as wound, physical therapy, occupational therapy, and speech therapy teams to develop individualized rehabilitation plans of care to ensure the greatest outcome for each patient’s recovery.
• Monitored patient progress, vitals, pain levels, and compliance daily.
• Educated patients and families on recovery processes, what to expect during inpatient stay + transfer to outpatient, wound care/wound prevention, medication administration, fall prevention, safe transferring, at-home care techniques, stroke prevention, traumatic brain injury recovery, catheter care, colostomy care, surgical drain care, PEG tube care and feeding instructions, outpatient resources, and pain management.
• Assisted in developing rehabilitation goals for individual patients.
• Documented patient information accurately in electronic health records systems. (Used CERNER)
