Job overview

  • Is the primary contact with the local Public Housing Agencies (PHA) and provides case management-based services (directly or through referral) necessary to ensure that veterans' care is coordinated with the VA medical center, community health, mental health providers, and agencies that serve the homeless population
  • The Social Worker conducts assessments and develops treatment plans and identifies goals of treatment based on these assessments and in collaboration with the veteran and family, where possible, and with the interdisciplinary treatment team
  • The Social Worker provides activities including field intervention, referrals for homeless veterans contacted in the community, shelters, and "on the streets", as well as for those who are referred by VA residential programs, medical center staff or outpatient clinic staff
  • S/he possesses the skills to offer psychosocial interventions and measures outcomes to interventions
  • Evaluates the veteran's environmental situation, including capabilities, and arrives at a sound conclusion regarding the veterans' vulnerability and the need for prioritization of admission to the HUD/VASH program
  • Coordinates community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs
  • Provide comprehensive case management services to Veterans
  • Provides crisis intervention services, seeks to address the cause as well as the presenting complaint, and coordinates family conferences and serves as liaison to family members
  • Provide referral, rehabilitation, and program admission activities that include referral to VA medical and mental health inpatient and outpatient treatment and to other VA services including VA benefits, as well as community-based social services and other non-VA entitlement programs
  • Implemented and started the first federal women employee mentoring group for federal employees desiring to further their careers with a goal to link senior employees to new employees or those desiring to switch fields within the VA medical Center
