Course: NURS-FPX4060: Practicing in the Community to Improve Population Health

Descriptions: As part of the coursework, I developed and implemented a Health Promotion Plan. The plan focused on addressing substance use among urban college students aged 18 to 25, emphasizing the risks associated with substance misuse and the potential health impacts on academic performance and mental health. I analyzed the health concerns related to substance use and collaborated to establish health promotion strategies and collaborative goals. 

We consulted various sources, including SAMHSA, Healthy People 2030, and academic literature, to inform the plan's development. The insights from academic literature were particularly valuable in understanding the importance of a health promotion plan in addressing substance abuse issues and promoting overall well-being in the community.

Key Achievements: I demonstrated an understanding of the health risks associated with substance abuse and the importance of prevention strategies. I collaborated effectively with peers and faculty to establish goals for improving population health in a community setting.

Professional Development: I attended annual provider conferences and obtained a certificate to complete reassessments for the state Medicaid program for chore services.

Excel Home Care- As the Director of Nursing and Business Development Officer, I directed and oversaw all nursing operations within the home care division, ensuring the delivery of high-quality, patient-centered care to clients in their homes. My work led to the development and implementation of clinical policies and procedures that ensured compliance with state, federal, and local regulations. I managed a team of nurses and other clinical staff members, providing training, performance evaluations, and professional development opportunities. I also coordinated patient care plans, working closely with physicians, therapists, and social workers to ensure comprehensive and cohesive care. My regular audits and quality assurance checks maintained high standards of care and addressed any areas for improvement. I liaised between patients, families, and healthcare providers to facilitate communication and resolve issues or concerns.
