Worked with multidisciplinary team on identifying ground level factors causing discharge delays and worked on improving EPIC and physical communication between team members and with patients and families to get safe and early discharges efficiently. Improved audio-visual communication regarding patient information towards discharge planning. Significantly reduced PT, OT and speech consultation to divert workforce towards efficient consultation in timely manner to patients who needed. Improved Interdisciplinary rounds quality with education on improvement on information transfer early in day to improve efficacy in early discharges to meet patients’ needs early. Many other goals achieved with this committee.
Been member on this multidisciplinary committee run by Department chair to discuss patient cases reported by any hospital team member to see clinical guidelines met during patient care and to thoroughly instigate cases and help with root case analysis and develop new work flow for future.
