• Initiates actions designed to prevent, minimize and/or correct potential or actual safety hazards to residents and/or staff. • Assist with podiatry, dental and vision screening visits and documents outcomes appropriately. • Perform customer service function making unoccupied beds, passing linen, passing nourishments and ice, stocking supplies, transporting residents, passing and collecting trays, recording meal percentages. • Answer call bells promptly and refer to appropriate caregiver assisting. May assist NA licensed staff with resident transfers for care purposes. • Serve as a transporter for Therapy Services. • Commit to high quality, safe work; encourage others to have similar standards. Identify processes/systems that could lead to inefficient practices, unsafe conditions, errors or adverse events. Avoid shortcuts to ensure quality and safety measures are not circumvented. Encourage patient and family involvement in processes involving patient safety. Understand that patient safety is everyone's responsibility. Assume responsibility for personal education and performance related to patient safety; report unsafe conditions or events immediately; speak up in a non confrontational and educational manner when seeing a safety breech; develop or participate in a project or action plan to reduce risk or mitigate harm for patients, as appropriate. • Direct residents and families to appropriate resource person for resident care information. • Assist in keeping resident's room and equipment clean and orderly, including dressers, closets, and wheelchairs. • Document resident information daily in the computerized documentation program. • Serve as safety sitter as needed for residents to observe patient behavior and maintain patient safety. • Know and follow all organizational/departmental policies/procedures. • Communicates with resident, family and other members of the health care team (verbal, non-verbal and written). • Actively practice behaviors associated with SH Service Excellence expectations.
