Seeking part-time clinical social work position working with individuals, families, and children.
Complete mental health and suicide assessments including diagnosis in the Emergency Department and in the inpatient Behavioral Health Unit. Provide medical social work services in Oncology, Labor and Delivery, Surgical Floor, and NICU. Provide individual and group therapies in the inpatient setting. Coordinate discharges with multidisciplinary teams and locate community resources. Develop treatment plans with patient and families. Assist patient and families with Advanced Directives and Living Wills. Assist patient is applying for Entitlement programs and locate community resources. Licensed Clinical Social Work Supervisor.
Conduct clinical assessments and clinical peer reviews. Coordinate and complete community based mental health assessments and assessments for substance abuse treatment programs. Determine clinical placements and continued stay reviews. Intensive case management. Crisis intervention response and services. Utilization management and coordinate care with community providers. Assess and coordinate care and treatment for involuntary clients whom are on probation or parole in Idaho.
Responsibilities include completing diagnostic assessments, treatment plans with individuals with persistent and severe mental illness and substance abuse disorders. Complete Designated Examinations on individuals on a mental health hold and provide testimony in court hearings. Complete PASS-R assessments (nursing level of care) and 19-2524 (criminal) and 18-212 (competency restoration) assessments. Coordinate with State and local hospitals in regard to admissions and discharges. Provide supervision of a multidisciplinary team in the delivery of clinical and treatment services; hiring staff and coordinating workloads. Complete mental health assessments and meet with walk in clients. Provide clinical supervision to master level social work staff in Region IV.
Receive and process referrals, complaints or requests for service and conduct assessment and fact-finding interviews of child abuse or neglect to obtain and assess mental, physical, economic, employment, environmental and social status of client/family. Prepare written intake reports based on initial interviews with referents and subjects of assessment and draft case plans related to child abuse/neglect issues with recommendations for case-related service decisions. Draft case plans related to child abuse/neglect cases and participate in interdisciplinary team staffing to formulate and/or modify treatment plans. Provide social services, support and change-oriented activities to preserve family unit and promote client self-determination. Prepare case record notes attesting to engagement with clients and results from specific services. Research, obtain and make referrals to community resources and provide court-related services including court testimony, court reports and other legal documentation and mediation, negotiation and implementation of agreements. Prepare court reports, affidavits, testimony, orders to remove the child, etc for required court proceedings. Consult with experienced social workers, law enforcement, court personnel, community resource workers and school staff.
Delivery Driver: Pick up and deliver freight, safe driving skills, time management, multitasking, customer service skills, attention to details, and communication abilities. Education requirements include a high school diploma and a commercial driver's license. Unload and load freight on aircraft.
Responsibilities included providing service coordination to families with children who have developmental disabilities or severe emotional disturbance. This included all areas affecting the child and/or family such as medical, educational and community-related. Served as a single point of contact for adults with chronic mental illness and dual diagnosis, assisting individuals identify, obtain and coordinate needed health, advocacy, and social services, coordinating with multiple social and health providers, ensuring quality in work performed, maintaining contact with clients on a regular basis, advocating on behalf of clients for appropriate community resources, overseeing implementation of service plan, facilitating periodic and annual reviews of plan, and determining continued appropriateness of the service plan or need for addendums to existing plan.
