I monitor patients that have many different diagnosis's once they are discharged home. Patients are monitored on Biofourmis platform where they take vitals and Im able to see them. I prepare the patient virtually by asking questions and concerns so they can be seen virtually by the virtual care provider. Also I have to use critical thinking skills since these patients are at home and escalate appropriately.

As a Care partner I had performed many duties. I did care coordination duties and followed patient under Transitional Care Management. I followed patients were discharged from the hospitals. I followed these patient for a total of 30 days. I would make initial calls and make sure patients have everything they needed such medications, DME, and making sure all gaps were closed with home health companies.

As a wellness coordinator. I focused on a different populations of patients. My job was to work on spread sheet that was generated by different insurance companies. Make sure patients were up to date with preventive care such as colonoscopy, mammogram, bone density test, and diabetic eye exams. Also I performed medicare wellness visits with physicians and letting them know what preventive care was missing.

While working in the internal medicine clinic. I interviewed patient about current problems, medications, and allergies. Also reviewed medical record for pertinent previous and current information. Also I performed patient treatment including administrating medications (under the supervision of the physician and registered nurse. Then I performed routine immunizations, nebulizer treatments, and visual acuity testing, administer and read TB test. Also facilitate and assist in coordination of patient referrals.

Managed care for adults with a wide variety of diagnosis. IV starting and therapy. Also lab draws. Tracheostomy care as well. Also developed patient care plans including assessments and nursing diagnosis. Then I managed care of post operatively patients. Also medications administration. Performed patients and families on proper discharge instructions and teaching.
