Provide Clinical Pharmacy Services to patients receiving Anticoagulation Therapy through the Centralized Anticoagulation Service Hub (CASH)

• Currently providing anticoagulation management for patients across 18 different VA sites
• Close Laboratory value monitoring
• Accurate Medical chart documentation
• Conduct Patient interviews and chart reviews
• Provides excellent customer service
• Collect and enter outside lab results and outside medical records as needed
• Provide Telephone logistical support
• Assist with medication refills and renewals
• Place and sign non-medication orders per policy
• Manage alerts, emails, voicemails, messages, faxes (incoming and outgoing)
• Perform administrative and clerical tasks
• Answer questions and address concerns or complaints related to anticoagulation management.
• Work with different Pharmacists throughout the day, triaging pt's accordingly
• Proficient in the use of Microsoft Windows Programs , CPRS, VistA, EHR, Joint Legacy Viewer (JLV), etc
• Use of both Warfarin and DOAC dashboards

• Fully knowledgeable and proficient in operating and navigating through Hines VA computer software systems, such as Vista, Computerized Patient Record System (CPRS), VISN 12 Phone Message Manager and Consolidated Mail Outpatient Pharmacy National Web Application (CMOP)
• Knowledgeable in federal and state regulations and assisted the pharmacists to the maximum extent
• Demonstrated initiative and self-motivation to prevent/resolve customer service problems or complaints
• Worked closely with pharmacists and used medication input software to safely and accurately dispense medication via Hines Intellicab system
• Efficiently answered multi-line phones and processed a high volume of order request from patients, nurses, doctors, and pharmacists via Vista, CPRS, and Message Manager software systems
• Established or maintained patient profiles, including lists of medications taken by individual patients
• Supplied and monitored robotic machines that dispense medicine into containers and label the containers
• Packed and mailed medication and medical supplies out to patients via USPS and UPS.
• Utilized Microsoft Office/Power Point Word and Excel to log medication and patient information

• Trained and advised other personnel in the operations of Pharmacy Systems and Point of Sale
• Fully proficient in all aspects in regards to inventory control (including ordering, receiving, updates, returns, recalls and claims)
• Assisted in various projects and promotions to increase sales and maximize profit

• Maintained good pharmacy department appearance (inside and out) and insured proper mylar and signage
• Provided friendly customer service on the phone and at the prescription drop-off and pick-up counters
