I have more than ten years of experience providing care coordination services to individuals living with Serious Mental Illness (SMI), substance use disorders (SUD), people living with HIV and people with chronic medical conditions. From an early age I committed myself to the delivery of care to vulnerable populations. I have demonstrated this by volunteering and obtaining employment in substance abuse centers and organizations servicing people with disabilities. Through my years of experience in Buffalo New York, I have gathered a good understanding of the healthcare system, services, and resources in the community. I have strong communication and interpersonal skills that have allowed me to easily build rapport with people of different socioeconomic backgrounds in the United States Army, as a teacher, a care giver, an administrative assistant, and a care coordinator. It is part of my natural character to be a strong team player. I like to help my team reach its objectives and support my team member's overcome difficulties in order to develop a strong, united and successful team. I often volunteer and take on added responsibilities when needed and show gratitude when other show the same support to me. I have the ability to assess complex situations, identify needs, and develop appropriate action plans. This ability has allowed me to be an asset as a HARP Senior Care Coordinator at Cinq Care for three years. Since my years of college education and work experience, I developed strong organizational skills and the ability to prioritize, manage time effectively, and meet deadlines. This is shown by the honors I have been awarded and the timely execution of my duties in care coordination and managing caseloads of up to 83 cases. I execute my obligations with care, compassion and a sense of duty. Service to others is my vocation in life.
