SENIOR CASE MANAGER

Concern for Independent Living
  • See clients at least 2-5 times a week at their homes. -Conduct Medication Counts with the clients. -Complete monthly fire drills at the client’s homes. -Work with the clients on their goals in order for the client to move into a more independent setting. -Write Progress Notes as well as Billings for Medicaid. -Conduct Collateral contacts with the client’s providers monthly. -Have Recreational Trips/Client Advisory Board meetings with the clients. -Write the client’s Service Plan’s/Service Plan Reviews every 90 days. -File paperwork into the client’s charts. -Take clients on appointments as needed. -Assist clients re: Social Security income, Food Stamps, SCAT Transportation, Medicaid Transportation, Medicaid/Medicare health insurance. -Conduct trial visits as well as move ins for new clients. -Complete Functional Assessments for clients every six months or 21 days after a new client moves into the program
  • Also update Functional assessments whenever the client is working on a new objective/Goal. -Conduct a Utilization Review for clients once a year and for new clients six months from when they were first admitted into the program and then yearly after that. -In charge of the hotline in case of any emergencies -Oversee the other Case Manager’s when Supervisor is on vacation or out. -Supervise all Interns and Peer Specialists
