Provide assessment, education, case management and counseling to patients diagnosed with End Stage Renal Disease, ESRD, and on dialysis. Adjustment to and acceptance of having s chronic disease is difficult emotionally and a necessity if the patient is to live the best quality of life they can while receiving treatment for their disease. Varied therapeutic approaches were employed such as cognitive behavioral therapy, motivational interviewing, interpersonal therapy etc. More often than not empathetic listening and seeing that basic needs were met were most helpful.
