Same Duties as with BSC WV

• Deliver skilled infusion therapy in patient homes via Peripheral IV, Implanted Port or PICC Line with medications including antibiotics, TPN, hydration, IVIG, biologic's, and specialty medications.
• Access and maintain central lines (PICC, midline, Implanted ports) with strict adherence to sterile technique and infection prevention protocols.
• Conduct comprehensive physical assessments and monitor for adverse reactions, ensuring prompt intervention when needed.
• Educate patients and caregivers on medication administration, line care, symptom management, and emergency response procedures.
• Provide emotional support and reassurance to patients coping with chronic illness and complex treatment regimens.
• Coordinate care with physicians, pharmacists, and case managers to ensure continuity and optimal outcomes.
• Maintain accurate and timely EMR documentation in compliance with regulatory standards.

• Managed care for diverse patient populations with acute and chronic conditions.
• Administered IV medications with precision and safety.
• Collaborated with interdisciplinary teams to develop and implement patient-centered care plans.
