Conduct individualized assessments for students to determine continued eligibility or discharge from services.

Provide services to students in grades 1-5 virtually

Provide ongoing and updated documentation as needed for progress reports, evaluations, IEP meetings, and collaboration with other team members.

Works closely with a multidisciplinary team in order to optimize patient success.

Completes documentation in order to monitor course of treatment.

Conduct informal/formal evaluations pertaining to a variety of communication needs

Develop and implement treatment plans individualized to clients across a variety of age ranges in order to address and support specific, functional needs

Consult and collaborate with associated professionals in order to optimize progress within plan of care

Work closely with families in order to promote optimal support and carryover within the home environment

Supervise graduate students within the clinical setting as needed

Provided virtual speech and language services for students grades 6-12 within the virtual academy

Completed daily progress notes and documentation required for evaluation reports and IEP meetings.

Attended IEP meetings and maintained continuous communication and collaboration with school educators and/or professionals to support student needs

Conducted screenings for incoming residents, residents with change in condition, and individuals with decreased independence with activities of daily living or meal management

Developed and implemented individualized treatment plans

Completed documentation to track progress and improve continuity of care

Collaborated with the corresponding medical team and family members in order to implement and follow through with plan of care

Provided in-services for the nursing staff related to dysphagia and overall meal management
