• Maintain professional autonomy while determining level of needs and dispatching appropriate staff to aide in providing 24-hour care for patients, family members, facility staff and other support persons.
• Triage high volume of incoming calls for high acuity needs in a timely and professional manner.
• Provide skilled nursing coordination of care, education and support to patients and caregivers, as determined by the current plan of care.
• Gather triage information and obtain personal history to determine appropriate care.
• Provide management and instruction for complex end of life symptoms.
• Initiate and maintain continuous care to provide for acute symptom management. Work directly with attending physician to receive and implement orders. Continually document effectiveness to promote patient care and accomplish symptom relief.
• Coordinate with attending physicians and additional hospice staff support to arrange visits for direct care, based on medical or emotional needs.
• Respond to medical or emotional emergencies or death of a patient by visit to provide immediate care and support.
• Report changes in patient status and coordinate care directly with physician and/or case managers.

• Assess patients during intake process and evaluate the healthcare needs, including medical, infectious disease, and psychiatric concerns, to assist Corrections Officers in determining housing needs.
• Complete History and Physicals on newly incarcerated patients, as required by State regulations.
• Perform assigned duties as part of a rapid response medical team for acute and critical patient needs that arise during incarceration.
• Administer interventions and medications in accordance with patient medical plans implemented by physicians.
• Complete comprehensive physical assessments to monitor patients for active withdrawal symptoms and administer medications based on symptoms and physicians instructions.
• Monitor patients’ response to medication regimen for psychiatric therapy and collaborate with provider on changes in behaviors.
• Complete controlled substance counts with shift change to ensure accuracy and secure storage of medications on premises.
• Complete “sharps” counts weekly to ensure accuracy and secure storage of instruments on premises.

• Complete comprehensive physical assessments to monitor patients for active withdrawal symptoms and administer medications based on symptoms and physicians’ instructions.
• Provide accurate and consistent reports to physicians and other team members.
• Provide patient education on prescribed medications and the importance of medication compliance.
• Provide patient education on addiction, recovery and coping skills.
• Complete controlled substance counts with shift change to ensure accuracy and secure storage of medications on premises.

• Perform wound care for patients with wounds including chronic diabetic ulcers, non-healing surgical wounds, chronic pressure ulcers and other trauma wounds.
• Assess and document skin alterations, assist in development of plans of care, and evaluate progress of specialized skin and wound conditions.
• Apply and maintain dressings such as negative pressure wound therapy systems, high compression therapy, pressure off-loading and other wound care dressings, as directed by physician.
• Assist with monitoring and calculating patient’s depth and decompression times for hyperbarics treatments, supervising the safety of patients and staff during treatment and maintaining the integrity of the chambers through inspection and preventative practices.

• Provide in-service training for accurate nursing applications and techniques to Licensed Practical Nurse students and Registered Nurse students.
• Provide direct guidance and hands-on training in wound care assessment and dressing application to Licensed Professional Nurse students and Registered Nurse students.
