Home Health Palliative Care Hospice Night Triage - Skills

  • Maintain professional autonomy while determining level of needs and dispatching appropriate staff to aide in providing 24-hour care for patients, family members, facility staff and other support persons.
  • Triage high volume of incoming calls for high acuity needs in a timely and professional manner.
  • Provide skilled nursing coordination of care, education and support to patients and caregivers, as determined by the current plan of care.

  • Assist with monitoring and calculating patient’s depth and decompression times for hyperbarics treatments, supervising the safety of patients and staff during treatment and maintaining the integrity of the chambers through inspection and preventative practices.
