I work as a Clinical Social Worker in the Palliative Care Department, at Bon Secours Maryview Medical Center and Bon Secours Mary Immaculate Hospital.
I conduct initial assessments on patients who have received a Palliative Care consult for end-of-life planning.
I have difficult conversations with patients and their loved ones, regarding their wishes regarding end-of-life planning and helping them to complete advance medical directives (AMD). The AMD establishes decision makers that the patient appoints.
I assist patients and their family members with completing DO NOT RESUSCITATION documentation, including Durable DNR or the POST form.
I offer support and counseling/therapy for patients and their loved ones as they come to terms with end-of-life decisions and planning.
I work in an interdisciplinary team that includes RNs, NPs and an MD who is also serving as our Medical Director. I work very closely with other physicians, throughout the health system to ensure a solid collaborative effort to help patient's transition towards their end-of-life or preplanning if death is not imminent.
