3. Provide ongoing support, guidance, and advocacy to help clients overcome obstacles and access necessary resources.
4. Coordinate referrals to internal and external services, including housing, employment, education, healthcare, and financial assistance programs.
5. Monitor clients' progress toward self-sufficiency goals and adjust service plans as needed to ensure success.
6. Empower clients through skill-building workshops, financial literacy training, job readiness programs, and other supportive services.
7. Maintain accurate and up-to-date documentation of client interactions, progress, and outcomes.
8. Collaborate with community partners and agencies to expand access to resources and support networks for clients.
