Provides direct and individualized nursing care to patients based on the application of
scientific nursing principles. In addition to general nursing care, responsibilities includes:
* Consults and coordinates with health care team members to assess, plan, implement and evaluate patients’ (both mother and infant) care plans
*Maintains a comfortable, safe and clean environment for mother and infant
* Assists mother with her hygienic needs
* Checks mother for any post partum bleeding
* Examines mother’s fundus
*Inspects c-section incision (if applicable)
* Assesses mother’s pain, if any
*Performs emotional status checks on mother, recognizing symptoms such as those related to postpartum depression.
* Monitors bonding between mother and infant
* Takes mother’s and infant’s vital signs
* Measures input/output of mother and infant
* Monitors mother and infant and reports adverse reactions to any treatments or medications being given
* Provides mother with breast-feeding tips
*Educates mother on sore nipple management
* Teaches mother bathing tips for infant
*Instructs mother on diaper change
* Weighs infant
*Performs heel sticks for bg’s of infant
* Keeps track of all feeds and diapers
* Bathes infant
*Involves and educates family/significant others in implementing best practices for mother and infant care
* Records all care information concisely, accurately and completely, in a timely manner, in the appropriate format and on the appropriate forms
*Completes required orientation as directed by facility
*Follows facility and OSHA safety rules and procedures while on assignment
* Respects cultural and religious practices of patients
* Upholds HIPAA regulations
Reviews patients charts and using Milliman guidelines to determine level of care to qualify them for health insurance coverage. Primarily dealing with CCS cases and approval in a higher tier hospital like Lucille Packard Children’s hospital.
*Provides direct nursing care for the newborn in accordance with established policies, procedures and protocols of the healthcare organization.
*Knowledge of normal lab and ABG values for neonates.
*Performs clinical nursing duties for neonates and infants according to policy and procedures.
*Maintains patient individualized standards of care.
*Performs neonatal assessment and provides appropriate intervention.
*Recognized physiological changes in the newborn and responds appropriately.
*Notifies physician promptly of non-reassuring newborn status.
*Cares for patients requiring newborn care, feeding, IV, medications, respiratory care including stabilizing and preparing for transport.
*Performs routine newborn blood draws for laboratory tests
*Operates all nursery/NICU equipment
*Strong knowledge base of medications used in the newborn period.
*Administered medications following policy and procedure demonstrating the 5 rights and knowledge of pharmacokinetics.
*Recognizes the developmental stages of neonates and infants and provides appropriate support to maximize patient’s developmental outcome.
*Provides age and culturally appropriate care.
*Starts IV on infants.
*Performs central blood draws on infants for laboratory tests.
*Performs urinary catheterization on infants.
*Assists with the insertion of umbilical line or chest tube.
*Assists with a spinal tap or intubation.
*Draws ABG’s from a central line.
*Manages and assesses the patient’s pain.
*Monitors, records and communicates patient’s condition as appropriate utilizing computerized documentation systems.
*In accordance with hospital policy, may insert I.V.’s, administer I.V. medications, and begin blood product transfusion.
*Observes and records patient reactions to medication, I.V. therapy, blood transfusion, and treatment.
*Provides and documents patient and family education as needed.
