Perform evaluations, assessments and treatment interventions for 8-12 patients daily in a Skilled Nursing facility.

• Participate in weekly interdisciplinary meetings along with family members to discuss patient's current level of function and make recommendations for discharge.
• Perform home evaluations to assess patient's safety, asses the use of durable medical equipment, and assess potential barriers that may pose a safety risk for patients anticipating discharge.
• Provide outpatient services to prior patients to ensure continuum of care.
• Perform Lymphedema evaluations and provide Complete Decongestive Therapy per physician orders.
• Managing scheduling for OTR/COTA staff.
• Efficient in wheelchair modifications, adaptations and positioning devices.

• Performed Home Health evaluations and assessments for individual patients within their home.
• Provide recommendations for Durable medical equipment in order to maximize independence with ADLS and increase safety.
• Design and implement home exercise programs in order to achieve goals and maximize independence with ADLS.
• Coordinated interventions with multidisciplinary treatment team including Registered nurses and physical therapists to maximize wellness and function.
• Skilled at working independently and collaboratively with interdisciplinary team to provide optimal care.

• Perform evaluations, assessments and implement treatment to patients with various musculoskeletal injuries including, but not limited to, rotator cuff repair, shoulder replacement and carpal tunnel repair.
• Knowledgeable in following physician's specific rehabilitation protocols for orthopedic surgeries.
• Proficient in the use of physical agent modalities including Electrical stimulation, Ultrasound, fluidotherapy, and Iontophoresis in order to facilitate recovery and aid in pain management.
• Fabricate static and dynamic hand splints per physician order.
• Participate in weekly interdisciplinary meetings to discus patient's current level of function and make recommendations upon discharge.
