As s student Nurse, I made my clinical rotation in the Riviera Health Resort, were I had opportunities to learn about:

• Treatment options, diagnosis information and home care techniques to patients and caregivers to continue care consistency.
• Coordination of Home Health Services and DMEs orders and authorization with Health Insurance,
  hospital CM and patients.
• Review of clinical, D/C orders, physicians’ orders and authorizations.
• Request and process authorizations.
• Schedule of patient SOC.
• Home Health visit note review.
• Maintenance of documentation of coordination with physicians, pharmacy, CM, UM and DME providers.
• Documented treatments delivered, medications and follow-up care.
• Scheduling and coordination of staffing providing services (RN, LPN, Therapist and HHA).
• SN services for assessment, monitoring, medication management and high-tech visits.
• Implemented care plans for patient treatment after assessing physician medical regimens.
• Followed all personal and health data procedures to effectively comply with HIPAA laws and prevent information breaches.
• Explained course of care and medication side effects to patients and caregivers in easy-to-understand terms.
• Educated family members and caregivers on patient care instructions.
• Administered medications and treatment to patients and monitored responses while working with healthcare teams to adjust care plans.
