I have been practicing in health care since I was 16, starting with being a CNA in a dementia unit. I had the opportunity to become a CNA at an early age from a program offered in High School at Central Campus. Once I graduated I went on to get my RN. I worked in a few areas while obtaining my BSN. I continued to work as a RN and eventually went back to become an ARNP. I was able to practice with Mercy One with Geriatrics specializing in Skilled/Long Term Care/End of Life until June of 2023, when they shut our clinic down.
I am an open and honest person. I feel I am a great team player and easy to get along with. I dedicate my heart and soul into caring for my patients, advocating what is best for them. I am excited for this opportunity, as it seems to be very similar to my job at Mercy Geriatrics, where I would still be working today if it was an option.
Work with primary care providers in LTC settings. Collaborate with pcp, nursing home staff, and all members of the patients care team.
My overall goal is to reduced hospitalization rates by following members closely, starting treat in place options. If they do go to the hospital I follow them as well.
