• Referring patients to Lung Nodule Clinic
• Calling patients with results

• Updating/communicating with PCPs

Manage Medical Oncology Nurse Triage Line by:

• Assessing patient symptoms and making appropriate recommendations, referrals, and follow up
• Documenting telephone encounters in Excellian

• Manage an interdisciplinary group consisting of Licensed Practical/Vocational Nurses, Certified Nursing Assistants and support staff to enhance the quality of hospice care.

• Conduct initial assessments of potential residents and implement service agreements with all residents upon move-in; create care plans; review and revise service agreements as needed or required.
• Monitor and maintain residents' medical records, ensure appropriate assessments are completed on time, appropriately document in records, and ensure care plans are in place.
• Complete fall investigation reports.
• Complete customized living input forms for residents on or applying to elderly waiver.
• Ensure all physician orders are signed and implemented within 24 hours.
• Maintain medication inventory.
• Supervise medication practices and medical equipment.
• Follow-up with physicians regarding medications and/or medical conditions.
• Assist in training to new employees as well as continued education for all staff; document training to assure compliance with local, state, and federal regulations.

• Ongoing treatment tracking (managed over 2000 patients).
• Assess patient records in Epic and iKnowMed for enrolling and tracking patients.
• Submit test requisitions for next generation sequencing.
• Mail copies of signed consent forms and opt-out letters to patients.
• Send result reports to treating physicians.
• Submit reports and consent forms to medical records.
• Request tissue blocks/slides from multiple pathology departments.
