For the last three years as long-term care social worker within a skilled nursing facility, I have effectively managed a caseload of 33 residents - all requiring a different level of needs emotionally, physically, mentally, and spiritually. As their assigned social worker, I communicate with my residents and their family members on a weekly basis to address their changing needs. This involves ongoing collaboration with medical providers, nurses, physical and occupational therapists, and our psychiatric nurse practitioner to implement interventions and services for additional support. These frequent changes to care needs are consistently updated in residents charts and care plan which provided additional growth for effective time management and prioritization skills. Often when working with this population, there are conversations regarding end of life care through hospice and palliative services. It is the job of the social worker to provide information to families in a sensitive and respectful way and initiate a referral with family desire. Though long-term care generally is the home to many of the residents who need more care than assisted living facilities or family members can provide; occasionally residents and their families would request discharge options and services that their loved one would be eligible for.
In addition to the aforementioned roles and responsibilities, I have been the lead long-term care social worker for the last year and a half and have overseen and supported three additional social workers on the long-term care units. The lead is responsible for implementing and modeling policies and procedures, assisting with training and staff improvement, but most importantly to provide additional support with difficult cases.
