• Per Diem temporary assignments to various skilled nursing facilities. Responsibilities vary based on assignment and census. Delivering skilled nursing care, medication administration and treatments to upwards of 22 clients.

• Providing in home skilled nursing care to two clients with developmental disabilities that included Autism (profoundly delayed) with explosive temperament disorder and Cerebral Palsy with bipolar disorder both non-verbal with Type I diabetes, celiac disease on precisely measured and calculated meal and insulin regimens one with a T-slim insulin pump. Other diagnoses included pancytopenia, common variable immune deficiency (requiring weekly sub-cut immunoglobulin treatments), TIA resulting in R hemiplegia (wheelchair dependent), SVC, critical airway with tracheotomy resulting in ventilator dependence with Gtube. Detailed assessments, ventilator/respiratory status monitoring, Gtube meds/care, trach care and ventilator monitoring, bathing, transferring, scheduling medical appointments, transportation via van equipped for power wheelchair, administration of all daily medications for both clients, meal preparation and all home care and shopping were my daily routine.

• Acute Dialysis RN providing unit and bedside treatments to AKI and AKI on chronic and peritoneal renal patients. Responsible for attentive and emergent care that encompasses interdisciplinary coordination with physicians, floor staff, laboratory and surgical areas. Timely collection and interpretation of labs/results and monitoring for safe and efficient care that includes, also peritoneal dialysis. Working independently, on-call/charge, in all phases of AKI/CKD care to include fistula, central line and PD catheter access and management. Adept and detail oriented set-up of machines as it relates to orders placed and integrity of dialysis machine to the safe provision of treatment. Currently working with Braun, but also experienced with Fresenius 2K. Well established and professional relationships with both patients and staff have allowed me to provide compassionate, detail-oriented and holistic care with support to family members.

• -Timely and accurate documentation in Epic including placement of appropriate orders as required/requested.
• -PMD/Match MD messaging services for communication with on-call physicians
• -Unit management to oversee flow of day, appropriate staffing for daily cases that includes: review of orders, labs, diet/meals, and coordination of other departments necessary for individualized patient care.
• -Care of Braun machines with Millennium RO systems through biomed monthly maintenance that involves collection and monitoring of water/machine cultures, disinfection and monthly PM care of RO and Braun through pH checks/balancing (quarterly) and mincare to the RO.

• Maintenance of a safety and efficiency in the clinic setting working with both acute and chronic dialysis patients. Achieved by teamwork, effective interdisciplinary communication and collaboration and appropriate delegation. Overseeing 20+ patients care through detailed management of medical treatments, access evaluations, chair side assessments, medication reconciliation, fluid management and holistic care through goal setting with use of Care Plans per protocol. Timely collection and review of labs and interdisciplinary communication to establish and execute interventions as needed. I tend to think outside to box as a critical thinker while using established policies, protocols and evidenced based research for achieving patient safety, clinic and patient goals. Water certified in CWP, all competencies and training up to date and completed in a timely manner. Compassionate and sensitive to patient’s and teammates diversity and needs.

• At this time our facility had broken up the role of this position, and a second RN oversaw the vaccination portion; however, I review this for accuracy before completing the monthly checklist. Currently I manage monitoring and surveillance of any infections through blood cultures and antibiotic treatments, along with follow-up to include staff/patient education, or proper delegation of such. I manged AORs and RCAs and all documentation relating to any of these events and provide, or delegate when necessary the necessary education/re-training needed based on my findings from the RCA tool.

• This was a diverse Medical-surgical unit that dealt mainly with, Colorectal and GI surgeries. Additionally, this unit serviced overflow for the disciplines of Gynecology, Gynecology-Oncology, Cardio-thoracic, Urology, Trauma and Ortho. The co-morbidities of this population included HTN, DM, CHF, AKI/CKD/ESRD, various cancers and psychological disorders. Working in collaboration with many disciplines and acknowledging a client’s entire medical history is the basis of my practice to aid in each individual’s safe and successful recovery. Within the main population my experience included critical thinking and assessment skills related to understanding/monitoring and/or responding to blood sugar and insulin protocols, blood pressure management and fluid/electrolyte balance and GI function as a whole. Additionally, my safe medication administration within this unit involves drawing/monitoring of labs. It is not uncommon on this unit for me to give blood, have heparin drips, IV antibiotics and give electrolyte replacement. Pain is managed on this floor daily through several modalities/combinations including, PO, PCAs and epidurals. Nutrition was another important aspect on this unit and many of these patients have special diets, receive TPN or tube feedings which involves my care of central lines and various forms of feeding tubes including G, G-J tubes and Freddie Millers. It is also not uncommon for our patients to have NG tubes. I have within my unit, and other units, managed trachs, percutaneous/JP drains, u-tubes, chest tubes, foley catheters/with irrigation, provided ostomy/urostomy care, managed general skin care for fistulas, surgical sites and worked with wound vacs. Other units that I have worked on include transplant, ortho, internal medicine, GI, neuro and general medicine. Importantly, I give “whole patient care” which involves compassionate communication advocating for my patients among disciplines. I pride myself in providing as much direct care and education to my patients and their loved ones so that they may return to their lives and continue their recovery in a safe and knowledgeable manner.
