● Review Comprehensive Plans of Care (CPOCs) for submission to the regulatory authority including any necessary revisions to the plan of care.
● Make the initial contact for new linkages.
● Review ten percent (10%) of each Support Coordinator’s case records monthly for completeness, compliance with these standards, and quality of service delivery
● Conduct individual face-to-face sessions with each Support Coordinator to review individual cases every week.
● Attend and participate in weekly staff meetings.
● Manage the assignment of caseloads in collaboration with the Program Director
● Accompany Support Coordinators at least annually on random home visits with participants.
● Ensuring support coordinators are being trained according to the ESL OAAS training guidelines.
● Direct all assigned staff to meet outcomes.
● Provide ongoing and annual review, assessment problem solving, and feedback regarding the delivery of support coordination services.
● Teach and monitor the application of person-centered principles and practices.
● Assure ongoing quality delivery of services.
● Review timesheets and mileage reports for accuracy
● Discipline and coaching of direct reports and at least one annual performance review for each.
● Mandated reporter. Any mandated reporters shall make reports immediately upon learning of incidents of abuse or neglect to Adult Protective Services and Elderly Protective Services
● Develop quality person-centered plans CPOCs according to the funder’s timelines
● Completion of monthly phone calls and quarterly visits monitoring plan implementation to ensure participant's personal outcomes are met.
● Respond to participant changes in needs, by completing necessary tasks to ensure revisions are done timely.
● Research, access, and assist participants in gaining access to needed waiver and other state plan services as well as needed medical, social, educational, and other support services
● Communicate and work with providers to ensure all services are delivered as outlined in your plan of care. ● Maintain all responsibilities and requirements regarding critical incidents.
● Meet with supervisor for weekly supervision, consultation, and participant needs.
● Attend and participate in weekly staff meetings.
● Attend and complete required training.
