• Discharge Planning

• Produces professional social work treatment plan and services to patients from newborn to elderly

• Provides direct clinical social work services to patients and their families in an interdisciplinary team approach

• Completes initial psycho-social assessments on assigned patients and provides psychotherapeutic counseling to individuals, family or groups as needed

• Gives clinical consultations as needed

• Provides patient centered advocacy

Makes referrals to community agencies, entitlement programs, and continuing ongoing education

• Participates in the evaluation, classification, and placement of all pupils with special needs

• Assesses a student in terms of personal and family history, social-economic and cultural differences

• Evaluates the configuration of factors within the home, community and school as related to a student’s current social and academic adjustment

• Participates on multi-disciplinary teams to facilitate an understanding of the educational abilities and limitations of the children referred for service

• Plans home visits or office interviews with family members to assess past and present history and development as well as family dynamics and interactions that are relevant to the child’s adjustment

• Maintains an ongoing relationship with families for the purpose of- sharing information regarding educational planning and programming for the student; assisting the family in utilizing appropriate community resources; and assisting the family and/or students to facilitate social adjustment

• Provide consistent, structured supervision (typically 1 hour/week) to support student learning and reflection.
• Guide students in applying social work theories, values, and ethical principles to field practice.
• Facilitate professional development in areas such as communication, critical thinking, advocacy, and cultural competence.
• Assign meaningful learning tasks and projects that meet the student’s learning objectives and required practice hours.
• Ensure student engagement with a variety of client systems, services, and interdisciplinary experiences as appropriate.
• Complete university-provided midterm and final evaluations of student performance based on CSWE competencies.
• Provide timely and constructive feedback to promote skill development and self-reflection.
• Maintain regular communication with the BSW program’s field liaison or faculty advisor.
• Promote adherence to the NASW Code of Ethics, confidentiality standards, and agency policies.
