Care Coordinator/Triage

• Triage calls using Clear Triage
• Submit PA to insurance companies.
• Coordinate care for patients, including scheduling appointments at other clinics and assisting with transportation when needed.
• Send referrals to specialists and follow up on referrals.

LPN

• Word under supervision of MD/NP
• Room Patients, take vital signs, weight, height
• Give immunization/medication as directed by MD/NP
• Clean rooms
• Run in house labs, hemoglobin, lead, blood sugars, urine, cholesterol, strep, influenza and Covid
• Do vision and hearing screens
• Assist providers with procedures
• Participate in Reach out and Reach and First Breath programs

• Work closely with and under the supervision of Dr. Larson in a private clinic setting
• Work duties include scheduling, medication reconciliation and renewals, patient triage
• Referrals and prior authorizations for medications and procedures
• Draw blood and run in office testing, manage anti-coagulation therapy per office protocol.
• Manage and administer immunizations
• Assist physician with medical procedures
• Client spectrum from pre birth to end of life

Med Record and Central Supply

• Review records for completeness, accuracy and compliance with regulations
• Retrieve patient medical records for physicians, technicians, other medication personal and other authorized persons
• Protect the security of medical records and ensure that confidentiality is maintained
• Enter data, such as demographic characteristics, history, extent of disease, medication and treatment orders, and diagnosis codes into the computer
• Process patient admission documents
• Review code status with incoming patients
• Arrange appointments, transportation and prepare required paperwork for appointments and physician rounds
• Monitor, order and stock supplies required for patient care
• Assist with obtaining proof of insurance and benefits for potential patients
• Assist with scheduling and replacing staff as needed
• Answering phone calls, directing calls to appropriate persons, taking messages and assisting callers as able.

LPN

• Worked for long and short term clients and contributed to team management by working as CNA with staff shortage
• Observe and assess patients, report change of condition to physician and implement orders as directed by MD.
• Provide patient care and treatments such as taking vital signs, blood draws, dressing wounds per orders, giving medications and monitoring for adverse reactions.
• Provider emotional, physical and spiritual support of clients
• Participated in family care planning

CNA

• Providing activities of daily living for patients including showering, dressing, toileting, walking, turning in bed.
• Provide assistance with meals as needed
• Provide assistance with transporting patients to activities and therapy.

LPN

• Provide care for short and long term patients
• Observe and assess patients, reports change of condition to physicians and implement orders as directed by MD
• Provide patient care and or treatments such as taking vital, listening to lungs, dressing changes per orders, giving medication and monitoring for adverse reaction
• Assisting MD on rounds
• Providing emotional, physical and spiritual support of patients
