The responsibilities are split between assisting the Director of Nursing by: helping resolve current patient issues, training new nurses and LPNs and laying out schedules; I also manage and often help in the intake and Care Coordination departments by accepting new patient referrals by coordinating with discharge planners and ordering doctors, scheduling clinicians for visits and coordinating with the patients, submitting insurance authorization requests and outbound and inbound orders.

• Provide complete care to home health patients, including physical assessments, medication and chronic disease management.
• Work with multidisciplinary team to carry out successful treatment plans for diverse acute and chronic conditions.
• Deliver oral medications, intramuscular injections and IV fluids to mitigate or resolve patient symptoms.
• Took samples such as blood and urine to complete diagnostic and routine assessment tests.
• Provide wound care to acute and chronic wounds coordinating the care with wound care clinics or Primary care providers.

• Provide emergency care for patients suffering from MI, Strokes, Cardiac Arrest, Trauma, Abdominal pain, overdoses and many other episodes. 
• Establish IV's, urinary catheters and IO's, orthopedic splinting, wound care, medication administration, assessing and monitoring vitals, and triage.
• Providing compassion, comfort, high quality care and reassurance to patients and their families.
• Collaboration and communication with ER Dr's,  nurses, and other departments such as Respiratory, Radiology, etc. 

• Performed psychiatric assessments of patients on mental state and overall well-being as well as physical needs as well as discussing coping methods.
• Explained details of mental illness and medication administered to patients' families.
• Administration of medications, reassessment of response to those medications and monitoring of vital signs.
