This role receives assignments from a diverse population of Dual Special Needs members statewide who require complex case management and follow up. Responsibilities include assessment of needs and development of a comprehensive care plan with a quick turn around time. Assess and mobilize family and community resources to meet identified needs. Assist members needing help with long term care to determine eligibility and register for the Home and Community Benefit Waiver and coordinator their services.

Conducted comprehensive needs assessments for children, adolescents and adults in the Medicaid Centennial Care Program, Long Term Care community-based agency and self-directed programs. Development and implementation of a comprehensive care plan to meet the Centennial Care, NCQA and United Health Care requirements. Secured medical care, behavioral health services and appropriate referrals for clients by working with community resources and contracted providers. Responsible for time-sensitive deadlines determined by Medicaid Centennial Care contract requirements. Maintained excel spreadsheet with time-lines of each member on case load.

Supervised employees in a state operated centralized call center for reporting suspected child abuse and neglect. Supervisor duties included the screening of child abuse and neglect reports, assigning a priority and assignment. As a call center supervisor duties also included employee hiring, training, and evaluation. As a call center it was necessary to manage customer service and assure quality measurements were met. Other duties included expertise in the area of social work with a background in child welfare, on call work, and collaboration with external and internal partners including law enforcement partners throughout the state.

Investigate child safety via in depth assessments of the family's circumstances, strengths, needs and risk factors. Develop a trustworthy relationship with families in crisis in a variety of settings including the office, home, school, hospital and in the community. Educate and make recommendations for services. Help access services and follow up. Testify and make professional recommendations to the court based on my investigation and assessment. Complete written reports and documentation. Participate in family meetings.

Engaged families in their homes and in the office to assess needs and strengths, develop and implement treatment plans. Assisted families to move towards positive outcomes, permanency and stability. Collaborated in multi-disciplinary teams to reduce recidivism and protect children and families. Provided written court reports and attended and testified in court proceedings. Advocated for the best interest of the children and families served.
