  • With the program planning team and Individual, develop, implement and monitor the residential habilitation plan and IPOP
  • Implement the Individualized Service Plan (ISP) as developed for each consumer by his/her family advocate and Service Coordinator
  • Responsible for ensuring that the consumer’s financial matters are handled within agency procedures, including management of the personal needs
  • Responsible for the scheduling of direct care staff ensuring that the number and types of staff assigned are adequate to meet the needs of the Individuals
  • Become certified in medication administration and maintain certification on an annual basis
  • Complete monthly paperwork accurately (i.e
  • Attendance records, petty cash, monthly receipts, personal allowance records, etc.)
  • Ensure that the agency vehicles are maintained and required transportation paperwork is complete, per policy
