• Utilize the occupational process of gathering information from the patient, medical records, and assessment to establish a patient centered plan of care
• Develop and provide therapeutic and occupational-based interventions to promote increased participation in activities of daily living
• Complete thorough documentation of patient care including evaluation, progress reports, patient/family education, and discharge summary.

• Evaluated patients and collaborated with patient, family, and health care team to determine appropriate level of support required upon discharge
• Educated and trained patients on use of adaptive equipment to promote independence in daily activities following surgery
• Collaborated with treatment team to determine appropriate discharge location given physical and cognitive abilities, as well as considered support system, discharge environment, and need for specialized care
• Facilitated early mobilization to prevent further decline while medical condition improved.

• Evaluate and treat a wide range of conditions including Chronic/acute pain, total joint replacement, functional decline, arthritis, CVA, TBI, and Neurodegenerative Conditions
• Cultivate caring relationships with patients through therapeutic use of self to facilitate recovery and health
• Supervise three OTAs and manage a caseload of 10-30 patients using daily collaboration and clear communication to ensure high quality of care
• Educate patients and family members regarding diagnosis, injury prevention, and therapeutic treatments.

• Plan and conducts individualized occupational therapy treatment sessions along with collaboration with a multidisciplinary team for short-term and long-term patients with varying diagnosis to help maintain, restore, or improve function in meaningful and everyday activities
• Provide patient and family education, organize daily documentation and update progress notes and discharge notes within 24 hours.
