Work collaboratively with ACS and CUNY to implement evidence-based initiatives/practice models.
Supervise 4 case planners and 30 families (50 children) in efforts to achieve permanency for each child.
Assist in weekly supervision to establish appropriate treatment plans, tailored to each child and birth parent.
Supervise four case planners and 40 families, providing oversight to help meet each family's individual service needs.
Train and develop skills for case planners when making safety and risk assessments, and proper documentation of family/client interaction.
Monitor timeliness of progress notes and FASP submission in accordance with program/ACS standards and guidelines.
Ensure that appropriate services are rendered to 20 children (birth-21) and their caregivers by meeting with clients on a weekly basis. Make changes and assessments to their service plan on an ongoing basis.
Develop and review treatment plans in consultation with both children and caregivers while assessing the quality of
services provided.
Advocate for clients by providing service referrals for birth families, and children as part of goal planning.
Prepare and present detailed permanency reports representing the agency to ACS and Family Court of the City of New York.
