• Knowledge of drug and alcohol dependency, mental health, family systems and 12-Step/recovery resources in the community.
• To build a collaborative support system with community employers, agencies and organizations, employing eligible clients as they move into the transitional period.
• To build a collaborative partnership with governmental housing agencies to readily respond to client needs for housing.
• To confront behavioral changes, identify a crisis, and recognize when additional assistance is necessary.
• To write and verbalize clear statements, summarizing clients job and housing progress to primary counselor and the treatment team.
• To write letters and progress reports to Department of Children and Family Services and the court system regarding clients employment and housing as deemed necessary for family re-unification.
• To Initiate, complete and follow up on the admissions, assessment, and referral process.
• To Communicate in a professional manner, obtain accurate information, and make appropriate placements in BHS facilities and appropriate referrals into other treatment programs.
• To note documentation in MyEvolve system.
• Basic understanding of computer operations; Windows and word processing (MS Word and Exel)
• To arrange transportation for clients from one facility to another.
• To demonstrate ethical and professional behavior.
• To convey respect for cultural and lifestyle diversities of clients and staff.
• To educate and link patients to MAT services. Submit referral and assist patient with any and all telehealth appointment with MAT team and MAT doctors.
