The Integrated Medication Assisted Treatment (IMAT) Jail Case Manager job was a new position created in an attempt to decrease recidivism with the San Mateo County population stricken with substance use disorder. Case Managers job duties include interviewing & assessing clients qualifications for IMAT program. Once qualified, Case Manager was responsible for client safety plan development post incarceration. Post incarceration work included linkage to community housing partners, crisis interventions, accessing public benefits, medical services & creating health relationships with local law enforcement. Work mentioned above required transportation using county vehicles & submitting timely documentation to track all services provided to each client. Case Manager responsibilities also required meeting clients where they are at, which may include homeless encampments, jail & hospital emergency rooms.
• Responsible for compliance with title 22 CCL regulations and communication with Community Care licensing representatives. • Responsible for compliance with all City, State, and Federal laws governing abuse reporting, client care and labor laws.
• Responsible for employee interviewing, hiring, recruiting and disciplinary action. Documented, developed, and implemented staff training.
• Responsible for ensuring care and supervision of residents are implemented according to client needs as documented in the pre-admission assessment for a 70-bed facility.
• Responsible for connecting clients to treatment services such as Drug & Alcohol treatment.
• Responsible for the care and supervision of all residents and supervision of personnel. Also responsible for developing an operating plan, procedures and budget and maintains all financial client and employee records.
