Coordinate the assessment and delivery of grief counseling needs and services (one-on-one, groups, and ongoing follow-up) to ensure timely and appropriate services are provided to family/caregivers. Documentation of grief counseling encounters. Plan for bereavement program development, expansion and refinement through annual evaluation of services. Provide pre-bereavement & bereavement support to hospice personnel coping with work related grief through one-on-one counseling, and referral to community resources. Provide bereavement information and referral services to callers from community resources. Expand and maintain community resource list as needed. Assist as needed in evaluating and planning services to meet hospice personnel support needs (ie., weekly support groups). Facilitate ongoing grief support at local facilities. Function independently but seeks and accepts guidance from other members of the interdisciplinary group/care team. Participate in hospice activities such as in-service education, annual foundation events, annual kids camp, attend hospice Idt meetings to determine the plan of care for patients/bereaved families/caregivers. Continuing education. Actively participate in quality assessment performances improvement teams & activities. Accept & perform other related duties and responsibilities as required (annual training to meet compliance). Submit & run monthly bereavement reports.
