• Cover 24 hours on-call provider line for multiple subacute rehab
  and skilled nursing facilities throughout New York State area.
  
• Manage care of chronically and acutely ill patients with hypertension, diabetes, Heart failure, COPD, asthma, renal failure, stroke, seizures, TBI, malignancy, chemotherapy, radiation.
• Triage patients, identify acute and emergency medical situations, order appropriate interventions, or refer to emergency room if necessary.
• Order and review labs, radiology reports, medications, consults, pharmacist recommendations.
• Prescribe medications, including narcotics, controlled substances, IV medications, and IV fluids.
• Perform chart reviews to monitor progress and adjust treatment plans.
• Handle vital sign alerts from RPM system.
• Communicate concerns regarding patients' status to interdisciplinary team including on-site physicians, specialists, nursing supervisors, case managers, and social workers.

• Work in the Emergency Department at a 535-bed tertiary care facility, conducting 124,000 emergency service visits each year.
• Provide direct patient care as a member of Attending Physician led teams in the Emergency Department includes evaluation, primary management, and disposition of undifferentiated acutely ill and injured patients.
• Under direct supervision of collaborating physician, perform and assist with endotracheal intubation, lumbar puncture, central lines insertion.
• Perform basic procedures including laceration repairs, wound care, incision and drainage, fluorescein eye examinations, pelvic examinations, orthopedic splinting.
• Accurately document patient's medical history and SOAP note using EHR.

• Conducted patient assessments, collected medical and psychosocial history.
• Provided comprehensive primary and urgent care including diagnosing and management of acute and chronic illnesses.
• Ordered and interpreted diagnostic tests, such as laboratory work and imaging studies, to aid in diagnosis and treatment decisions.
• Prescribed appropriate medications and therapies, adhering to evidence-based guidelines and considering patient preferences and limitations.
• Monitored patient progress, assessed outcomes, and made necessary adjustments to treatment plans to optimize patient care.
• Maintained accurate and up-to-date electronic health records for all patient encounters.
