Provide comprehensive nutrition assessment, education, and case management for clients with a wide range of medical conditions, including but not limited to hyperlipidemia, hypertension, Type 1 and Type 2 diabetes, failure to thrive (FTT), cerebral palsy (CP), autism spectrum disorder (ASD), cardiac and renal conditions, gastrointestinal disorders, gastroesophageal reflux disease (GERD), food allergies, prematurity and micro-prematurity, hyperemesis gravidarum, anemia, polycystic ovary syndrome (PCOS), and mental health conditions.
· Assess, monitor, and manage clients receiving enteral (tube) feedings, ensuring appropriate nutritional interventions and formula recommendations.
· Consult with Registered Dietitian Nutritionists (RDNs) within the Nutrition Unit to coordinate care plans and optimize outcomes for mutual clients.
· Consult with Children's Special Health Care Services (CSHCS) nurses to determine appropriate therapeutic formulas and food packages based on medical diagnoses and nutritional needs.
· Consult with Early On nurses regarding feeding therapy referrals and interventions for infants and young children with feeding difficulties.
· Consult with Public Health Nurses to coordinate client care, provide nutrition guidance, and support interdisciplinary case management.with Registered Dietitians (RDNs),
· Consult with physicians, and other healthcare providers to develop and implement individualized nutrition care plans for medically complex clients.
· Provide breastfeeding education and support, including assistance with latch, engorgement, milk supply concerns, nipple shield fittings, loaner breast pump education and maintenance, and support for mothers with infants in the Neonatal Intensive Care Unit (NICU). Monitor breastfeeding progress and collaborate with International Board Certified Lactation Consultants (IBCLCs) through care conferences as needed.
· Obtain anthropometric measurements, including height, weight, and head circumference, and perform hemoglobin screenings to assess nutritional status and identify health risks.
· Develop and manage individualized care plans for high-risk clients, providing ongoing monitoring, education, and follow-up.
· Conduct nutrition consultations related to elevated blood lead levels, interpret WIC lead screening results, provide education on reducing lead exposure and improving nutritional status, and manage follow-up for high-risk cases.
· Perform quality assurance reviews for special formula requests to ensure compliance with program requirements and medical necessity.
· Complete quality assurance activities for the Nutrition Care Referral Data (NCRD) report, ensuring documentation accuracy and regulatory compliance.
· Deliver direct client services in a high-volume clinic setting by conducting nutrition assessments, providing individualized counseling, determining WIC eligibility, issuing appropriate food benefits, and coordinating referrals to community resources while maintaining efficient clinic operations
