• Provide social work services to consumers and their families; many who are experiencing complex bio-psycho-social issues, coping with a critical illness, physical or developmental disabilities
• Provide families, community agencies, medical teams, and service providers education on the implementation and utilization of SARC services, intervention plans, eligibility criteria for resources, and limitations to services
• Develop and help establish Individual Program Plans (IPP's) with families to support client in achievement of personal goals and identify possible areas of unmet needs
• Consider relevant economic factors, emotional, and bio-psycho-social when completing written psychosocial assessments to address child and family needs

• Developed and facilitated 6mo., annual and transition planning meetings with interdisciplinary team to ensure child progress, family support and monitor overall development for children under the Early Start program, within specific timelines
• Actively participated in staffing and interdisciplinary case presentations to contribute developmental and psychosocial information and to advocate on behalf of the client
