• As a Behavioral Health Care Manager, I helped facilitate access to mental health services for the disabled and elderly populations. In this position the average caseload sizes of 70 patients each month. Goals included coordination of care and help symptom management by engaging with not only the patient but their family and the community in which they live to give them optimal patient outcomes.
